How effective is fertility preservation for childhood cancer?

Cancer treatment can affect the cells that later make eggs or sperm. After puberty, egg, embryo or sperm freezing may be possible. Before puberty, ovarian or testicular tissue preservation may be discussed in selected cases, although the approach and future use are different. The oncology team must weigh the child’s treatment urgency, physical development and wellbeing.

Preservation does not guarantee future fertility or pregnancy. It creates stored material or tissue that may offer an option later, while some children retain fertility without preservation.

Ask for fertility counselling when the cancer plan is made, ideally before treatment begins. A paediatric oncology team can involve a reproductive specialist and explain sedation, procedures, storage and follow-up in age-appropriate language. Parents or guardians can share family goals while the child’s understanding, comfort and developing autonomy remain part of the discussion.

Request review if fertility was not discussed before treatment, or later if puberty is delayed, periods do not begin or stop, or there are concerns about testicular development. Seek urgent care for severe pain, fever, heavy bleeding or breathing difficulty during any procedure or medicine course.
