How an orthotic may help
The plantar plate helps stabilise a toe joint at the front of the foot. A tear can make push-off painful and may cause swelling, tenderness or a feeling that the toe is moving out of position. An orthotic can redistribute load away from the sore joint and may use a shaped area behind the forefoot to reduce stress during walking. Protect the forefoot while it settles
Use the orthotic only as advised and pair it with shoes that have enough room, a stable sole and limited pressure over the toes. Your clinician may recommend reducing impact activities, taping or temporarily limiting push-off while the tissue is protected. Check the skin and the painful joint after wearing the device. New rubbing, increasing swelling, toe crowding or tingling means the fit needs review; do not keep adding wear time to overcome those problems.
When an examination is needed
Seek assessment when forefoot pain follows an injury, walking becomes difficult, the toe appears to drift or swelling does not settle. Get prompt care for severe pain after trauma, a visibly deformed toe, inability to bear weight, or a foot that becomes cold, pale or numb. A persistent tear may need more than an insert, so worsening symptoms should not be managed by changing orthotics alone.
