Short answer

Custom orthotics may reduce patellofemoral pain for some people by changing foot mechanics, but they work best when matched to the individual and combined with rehabilitation.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

The link between feet and knee pain

Patellofemoral pain is felt around or behind the kneecap and can increase with stairs, squatting, running or prolonged sitting. Foot position and movement can influence how the lower leg rotates and how force travels through the knee. An orthotic can alter that load for selected patients, but it is not a universal solution and does not repair cartilage or directly strengthen the thigh and hip muscles. The knee should still be assessed for other causes of pain.

Using an orthotic safely

A clinician can assess your feet, gait, knee movement, shoes and activity demands before prescribing a device. Introduce it gradually and pair it with the recommended strengthening, mobility and load-management programme. Use supportive shoes and replace worn footwear. Keep track of which activities provoke pain and whether the device changes your walking comfort. When to seek care

Seek prompt care for a locked knee, major swelling after an injury, inability to bear weight, a hot red joint or sudden loss of strength. Arrange a review if pain is worsening, your knee gives way, or an orthotic causes new foot, ankle, hip or back symptoms. A different diagnosis or treatment plan may be needed.

Questions for your doctor

Ask whether foot mechanics contribute to your knee pain, what the orthotic is intended to change, which exercises should accompany it, and when the fit and progress should be reviewed.

Questions people ask

Are orthotics enough for patellofemoral pain?

Often they are one part of care rather than the whole plan. Exercise, gradual activity changes and appropriate footwear may be needed as well.

What if knee pain increases with the device?

Reduce wear and arrange a fit assessment. Increasing pain can reflect an unsuitable design, a shoe problem or a knee issue that needs reassessment.
