How effective is chemotherapy for bladder cancer?

Some chemotherapy is placed directly into the bladder after a procedure, while other medicines travel through the bloodstream. The choice depends on whether cancer is limited to the bladder lining, has grown into muscle, or has reached other areas. Treatment may be given before surgery, after surgery, alongside radiation, or to control more advanced disease.

Effectiveness is judged by the treatment goal. A procedure and bladder examination may show whether surface disease has returned; scans and symptoms may help assess disease elsewhere. Blood tests can check whether your body is ready for the next treatment. Side effects can include tiredness, nausea, changes in urination, lowered blood counts or infection risk.

Ask whether your chemotherapy is bladder-directed or systemic and why that route is suitable. Tell the team about kidney problems, hearing changes, neuropathy, infections and all medicines. Keep a record of urinary symptoms, temperature and side effects. Ask what the next assessment will be and whether surgery, radiation, immunotherapy or another approach is part of the plan.

Call your treatment team for fever, chills, burning or worsening pain when passing urine, inability to pass urine, persistent vomiting, new bleeding, or severe diarrhoea. Get urgent help for confusion, fainting, breathing difficulty, chest pain or sudden severe weakness. Do not wait for a routine appointment if symptoms are rapidly worsening.
