How effective is a blood test for gout?

A blood test can support a gout assessment, but it cannot confirm or exclude gout on its own. It commonly measures uric acid and may also check kidney function or look for another explanation for a painful, swollen joint. Uric acid can be within the usual range during an attack, while some people have a raised level without gout.

Gout develops when urate crystals collect in and around a joint, often causing sudden pain, warmth, swelling and tenderness. The blood result shows the amount of uric acid circulating at that moment; it does not show whether crystals are present in the joint. The timing of the sample, recent treatment and kidney health can all affect interpretation.

When the diagnosis is uncertain, a clinician may recommend taking joint fluid with a needle so it can be examined for crystals. Examination of the joint fluid is more direct than a blood result. The pattern of attacks and examination findings remain part of the assessment.

Arrange an assessment during a new or unexplained attack, particularly if the joint is very swollen. Do not start or change urate-lowering medicine based only on a laboratory result. Keep a note of when symptoms began, which joint is affected, medicines you take and whether you have had similar episodes. A clinician can decide whether repeat testing, joint-fluid testing or imaging is appropriate.

Seek urgent medical assessment for a hot, swollen joint with fever, chills, feeling faint or rapidly worsening pain. Infection inside a joint can resemble gout and needs prompt treatment. Get same-day advice if swelling follows an injury, you cannot move the joint, or you have significant kidney disease and a sudden painful flare.

Ask whether the result was taken during an attack, whether it should be repeated after symptoms settle, and what other causes of joint swelling need consideration. You can also ask whether joint-fluid testing would clarify the diagnosis and how kidney function affects treatment choices.
