What blood tests can and cannot do

Chronic fatigue syndrome is assessed from the pattern of symptoms, their effect on activity and sleep, and the absence of a better explanation. Blood tests may check for anaemia, thyroid problems, inflammation, infection, nutrient deficiency, diabetes, liver or kidney problems and other contributors. The exact tests depend on your history and examination.

A normal panel does not mean that your symptoms are imagined or insignificant; it means those particular conditions were not shown by the tests used. An abnormal result may point to a treatable alternative or an additional problem. Repeating tests is considered when symptoms change or the first assessment was incomplete.

Making the assessment useful

Before the appointment, note when fatigue began, what activity triggers worsening, how sleep affects you, and symptoms such as pain, dizziness, fever, weight change or concentration difficulty. Bring a list of medicines and supplements, including recent infections and relevant medical history. Ask how results will be explained and what follow-up is planned if the panel is normal.

When to seek care

Seek urgent medical help for chest pain, severe breathing difficulty, fainting, new confusion, one-sided weakness, sudden speech or vision changes, or thoughts of self-harm. Arrange prompt review for rapidly worsening fatigue, unexplained weight loss, persistent fever, significant bleeding or inability to keep fluids down. These warning signs need assessment even if earlier tests were normal.
