How effective is antenatal steroid injection for premature rupture of membranes?

When the membranes break before labour, fluid may leak and birth may happen earlier than planned. A corticosteroid injection is considered when the clinical team thinks preterm birth is possible and the potential benefit for the baby outweighs concerns for the pregnant person. It helps prepare the baby's lungs for breathing after birth; it is not a treatment that repairs the membranes.

Care also includes checking temperature, pulse, pain, uterine tenderness, the fluid and the baby's heart rate. Antibiotics or a change in delivery plan may be considered depending on the assessment. The decision is individual because infection, bleeding, labour and gestational age alter what is safest.

Contact your maternity unit immediately if you think your waters have broken, even if the leak is small or painless. Use a sanitary pad rather than inserting anything into the vagina, note the colour and smell of the fluid, and follow the hospital's instructions about eating, bathing and activity.

If injections are offered, ask when each dose is due and tell staff about diabetes, allergies or medicines. Keep monitoring instructions with you and attend every review, because the plan can change if labour starts or infection is suspected.

Seek urgent maternity assessment for fever or chills, foul-smelling fluid, vaginal bleeding, increasing abdominal pain, regular contractions, feeling faint or unwell, or reduced baby movements. These signs may indicate infection, labour or a problem requiring immediate treatment. Do not wait for a scheduled injection or appointment if they occur.
