How effective is allogeneic stem cell transplant for aplastic anaemia?

The aim is for donated blood-forming stem cells to settle in the marrow and produce red cells, white cells, and platelets. A successful engraftment may reduce the need for transfusions and lower the problems caused by low blood counts. Recovery is assessed with blood tests, marrow checks, and monitoring for infection or graft-versus-host disease, in which donor immune cells react against the body.

The result is not determined by the transplant alone. A close donor match, the recipient’s age and general health, prior transfusions, infections, organ function, and the cause and severity of marrow failure can all affect planning and recovery.

Ask the transplant team to explain why this treatment is being considered, what alternatives fit your situation, and how donor selection changes the plan. Before conditioning begins, discuss fertility, vaccinations, medicines, dental care, infection precautions, and the support needed at home. Keep a record of fevers, bleeding, breathlessness, and transfusions so the team can respond promptly.

During transplant recovery, contact the transplant unit urgently for a fever, chills, new cough, breathing difficulty, unusual bleeding, black stools, severe diarrhoea, a spreading rash, or yellowing of the skin. Do not wait for the next appointment: low white-cell or platelet counts can make infection and bleeding develop quickly.
