How effective is abdominal ultrasound for fatty liver disease?

The scan uses sound waves to create images of the liver and nearby organs. Fat may make the liver appear brighter than expected. This can support a diagnosis of fatty liver disease and may also reveal other findings, such as gallbladder changes or bile-duct widening.

A normal-looking scan does not rule out early or mild fatty change. Ultrasound also cannot by itself distinguish simple fat from liver inflammation or determine the stage of fibrosis. Your clinician may combine the image with symptoms, medical history, liver blood tests and a fibrosis assessment.

Ask what the report says about liver appearance and whether another test is needed. Follow any preparation instructions, which may include avoiding food for a period before the scan so the images are clearer. Take your medication list and mention alcohol use, diabetes, weight changes and previous liver problems.

If fatty change is reported, arrange follow-up to review liver enzymes and your wider metabolic health. Do not treat the scan as a measure of liver function; a clinician may recommend monitoring or further assessment based on the whole picture.

Arrange prompt medical advice for yellowing of the skin or eyes, new swelling of the abdomen, vomiting blood, black stools, confusion or severe persistent abdominal pain. These symptoms need assessment rather than reliance on an ultrasound appointment.

Ask whether the scan shows fatty change only, whether blood tests suggest inflammation, and how your risk of scarring will be assessed. You can also ask when repeat testing is appropriate and which changes to symptoms should trigger earlier review.
