How does hip resurfacing work?

Short answer
Hip resurfacing works by smoothing the damaged femoral head, covering it with a metal cap, and placing a matching component inside the hip socket.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Bone preserved
- Most of the femoral head and neck remain
- Implant design
- A cap covers the ball and a cup lines the socket
- Decision factors
- Bone quality, hip shape and activity goals
What the surgeon changes
During the operation, the surgeon removes damaged surface bone and cartilage from the ball of the hip but keeps most of the femoral head and neck. A shaped cap is fixed over the prepared ball. The worn socket is also prepared and lined with a cup, allowing the capped ball to move within it. The components are designed to create a smoother joint surface and reduce pain from bone-on-bone contact.
Hip resurfacing is not suitable for every person. Bone quality, the shape of the hip, age, activity goals and the reason for joint damage all affect the discussion. Hip osteoarthritis can damage both joint surfaces, but the operation chosen depends on the individual assessment.
Planning your recovery
Before surgery, review medicines, dental or skin infections and the exercises recommended for your muscles. Afterward, use the walking aid and weight-bearing instructions supplied by your team. Build activity gradually through rehabilitation, paying attention to hip strength, balance and controlled movement. Avoid high-impact or twisting activity until your surgeon confirms that the hip has healed and your movement is safe.
When to seek care
Contact your care team for increasing wound redness, fluid, fever, worsening groin pain or a new change in walking ability. Urgent care is needed for sudden breathlessness, chest pain, a markedly swollen calf, or a leg that becomes cold or numb. New persistent clicking or pain after activity should be assessed rather than ignored.
Questions for your doctor
Ask why resurfacing is being considered, how your bone quality affects suitability, which alloy is used, and how the choice compares with total hip replacement. Discuss expected weight-bearing, activity limits, follow-up imaging and signs of implant trouble.

Questions people ask
How is resurfacing different from total hip replacement?
Resurfacing caps the existing femoral head, whereas total hip replacement removes the femoral head and replaces it with a stemmed component as well as replacing the socket surface.
Can everyone with hip osteoarthritis have resurfacing?
No. The suitability of resurfacing depends on bone strength, hip anatomy, disease pattern and personal factors assessed by an orthopaedic surgeon.
