Why the signs can be missed

With age, thirst may not signal the need for fluid as strongly. Memory problems, reduced mobility, swallowing difficulty, vision changes or needing help to reach a drink can further limit intake. Fever, vomiting, diarrhoea, hot weather and some medicines can increase fluid loss. Dry mouth, dark urine, constipation, headache, dizziness, weakness or new confusion may be clues, but signs vary.

Dehydration can affect blood pressure, kidney function and alertness. A person taking fluid-restricting medicine or living with heart or kidney disease may need an individual fluid plan rather than simply drinking as much as possible.

What to do

Offer regular drinks and water-rich foods if swallowing is safe, using a cup or bottle that is easy to hold. Record what is taken if intake is a concern and ask whether support is needed at meals. During vomiting or diarrhoea, ask a clinician or pharmacist whether an oral rehydration product is suitable. Do not stop prescribed medicines or override a fluid restriction without advice.

When to seek care

Seek urgent help for fainting, severe weakness, new confusion, inability to keep fluids down, very little urine, rapid breathing, or a person who is difficult to wake. Contact a clinician promptly if dizziness persists, urine becomes much darker or intake remains low. Older adults can worsen without strong thirst, so a clear change from normal matters.
