How common is vomiting blood in oesophageal cancer?

Blood may appear bright red, dark red, or brown and granular like coffee grounds. It can come from bleeding in the oesophagus, stomach, or upper digestive tract. In oesophageal cancer, a lesion can bleed, but this symptom also has other important causes, including irritation, ulcers, tears after retching, and enlarged veins in the digestive tract.

The colour and amount cannot safely identify the source at home. Associated symptoms such as dizziness, weakness, chest discomfort, black stools, swallowing difficulty, or ongoing vomiting give clinicians important clues.

Do not wait for a routine appointment if you vomit blood or coffee-ground material. Seek urgent medical care now. If possible, tell the care team when it began, what it looked like, whether you have black stools, and which medicines or supplements you take, particularly those that can affect bleeding.

Avoid eating or drinking if you are actively vomiting and follow emergency-service instructions. If you feel faint or weak, do not drive yourself; ask someone to help or call emergency services.

Call emergency services immediately for a large amount of blood, fainting, confusion, cold or clammy skin, severe weakness, shortness of breath, severe chest pain, or continuing bleeding. These signs can indicate significant blood loss or another serious problem.

Once you are stable, ask what may have caused the bleeding, whether the oesophagus needs direct examination, which medicines should be reviewed, and what symptoms should trigger another emergency visit.
