How common is swelling in pregnancy with pre-eclampsia?

Some swelling of the feet and ankles is common in pregnancy because the body holds more fluid and the growing uterus can affect circulation. With pre-eclampsia, swelling may appear with high blood pressure and other signs that require assessment. Sudden swelling, particularly of the face or hands, is more concerning than gradual ankle swelling, but appearance alone cannot settle the diagnosis.

Your maternity team may measure blood pressure, test urine, ask about symptoms, and assess you and the baby. Pre-eclampsia can need review even when swelling is not prominent, so continue planned antenatal checks and report changes rather than relying on swelling as the only warning sign.

Tell your midwife, obstetrician, or maternity unit about swelling that is new, sudden, increasing, or affecting your hands or face. Follow the monitoring and treatment plan provided by your clinical team. Rest with your legs raised when comfortable and avoid prolonged standing, but do not use someone else's medicine or make major dietary changes without advice. Record when the swelling began and whether it changes during the day.

Seek urgent maternity advice for a severe or persistent headache, blurred vision or flashing lights, pain beneath the ribs, sudden swelling of the face or hands, significant nausea or vomiting, shortness of breath, or a sudden feeling of being very unwell. These warning symptoms need prompt assessment in pregnancy. If you have severe symptoms or cannot reach your maternity service, use emergency services.

Ask: Could my swelling be related to my blood pressure? What blood-pressure reading needs urgent action? Should my urine, blood tests, or the baby's wellbeing be checked? How soon should I be reviewed? Which changes should I report during the day or overnight?
