How common are skin blisters in cold sores?

Cold sores often begin with tingling, itching, or burning, followed by small fluid-filled blisters on or near the lips. The blisters can break, weep, and form a crust before healing. Episodes may recur in a familiar spot, but the timing and severity can differ.

Blisters around the mouth can have causes other than cold sores. A clinician may consider the location, sequence, pain, recurrence, and contact history before confirming the diagnosis, particularly when sores are widespread or unusual.

Keep the area clean and avoid picking the crust. Wash your hands after touching your face, do not share lip products, utensils, towels, or toothbrushes, and avoid kissing or oral sex while a sore is present. A pharmacist or clinician can advise whether an antiviral treatment is suitable, especially when started early.

Use a simple protective balm if the skin is cracking, and avoid products that sting. Take care not to touch your eyes after contact with the sore; hand hygiene matters throughout the episode.

Arrange care if sores are near the eye, very painful, extensive, repeatedly recurring, or slow to heal. Seek prompt help for eye pain, light sensitivity, vision changes, fever with widespread sores, or sores in someone with a weakened immune system or a newborn contact.
