How common is shortness of breath in heart failure?

Heart failure can make it harder for the heart to meet the body's needs and may lead to fluid building up in the lungs or elsewhere. Breathlessness may be noticed during activity, when lying flat or when waking at night. Some people also develop ankle or leg swelling, tiredness, a persistent cough, or a rapid change in weight related to fluid. The particular pattern and pace of symptoms are useful information for the clinical team.

Take heart medicines exactly as prescribed and follow any individual guidance about fluids, salt, activity and weight monitoring. Record a clear change in breathing, swelling, sleep position or ability to complete usual tasks. Contact your heart-failure team or clinician if symptoms are getting worse, because treatment may need adjustment. Do not stop or alter prescribed medicine on your own unless a clinician tells you to do so.

Call for urgent medical help for sudden or severe breathlessness, chest pain, fainting, blue or grey lips, new confusion, sweating with marked illness, or coughing up pink frothy fluid. Promptly contact a clinician for a noticeable worsening in breathlessness, new swelling, increasing difficulty lying flat, or breathlessness that wakes you from sleep. These changes may need assessment even if they ease after resting.
