Placental abruption happens when the placenta separates from the uterus before birth. Pain may be sudden, constant, or felt as a tender, firm abdomen. Vaginal bleeding can occur, but it may be absent when blood stays behind the placenta. Tightening, back pain, reduced fetal movement, dizziness, or feeling faint can add to the concern. This symptom pattern requires clinical assessment because the mother and baby may need monitoring even when the amount of visible bleeding seems small.

Stop exertion and contact your maternity unit or emergency service immediately. Do not drive yourself if the pain is severe, you feel weak, or bleeding is present. Note when the pain began, whether it is continuous, any bleeding or fluid loss, and changes in fetal movement. Keep your antenatal records and medication list available. A clinician may check your vital signs, abdomen, bleeding, fetal heart rate, and contractions before deciding on further care.

Seek emergency help for severe or worsening pain, a rigid or very tender abdomen, heavy bleeding, faintness, breathlessness, collapse, regular contractions, or reduced fetal movement. Treat these as urgent warning signs at any stage of pregnancy. Do not wait for bleeding to appear or for pain to settle, because placental abruption can present without obvious external blood.
