How common is severe abdominal pain in pregnancy in HELLP syndrome?

Severe Abdominal Pain in Pregnancy is a common presentation of HELLP Syndrome. The pain is often felt in the upper right abdomen or below the ribs and may come with nausea, vomiting, headache or feeling very unwell.

HELLP Syndrome is a serious pregnancy complication involving red blood cell breakdown, liver irritation and low platelets. Upper abdominal pain can reflect liver involvement. It may develop during pregnancy or after birth, and some people have high blood pressure or visual symptoms while others do not notice those signs first.

Severe abdominal pain has other possible causes, but in pregnancy it needs prompt clinical assessment. Blood pressure checks, blood tests and fetal monitoring help clinicians assess your health and the baby's wellbeing.

Contact your maternity unit or emergency service immediately for severe upper abdominal pain during pregnancy or after birth, especially if you also have headache, vision changes, nausea, vomiting, swelling or reduced baby movements. Say that you are pregnant or recently gave birth and describe the pain clearly.

Do not wait for a routine antenatal appointment. Bring your medicines and pregnancy records if they are nearby, but leave promptly. Hospital assessment may include repeated blood pressure readings, laboratory tests and monitoring of the baby.

Call emergency services for severe or worsening pain with faintness, collapse, breathing difficulty, confusion, heavy bleeding or extreme weakness. These symptoms can indicate a serious complication requiring immediate treatment. Urgent care is also needed if symptoms begin after delivery.
