How common is nipple discharge in Paget disease of the breast?

Nipple Discharge is a common presentation of Paget Disease of the Breast. This condition affects the nipple and surrounding areola and may occur with changes that resemble irritation or eczema.

Possible changes include persistent redness, scaling, crusting, itching, burning, soreness, nipple flattening or inversion, and clear or bloody discharge. A lump deeper in the breast may also occur, but its absence does not explain away a lasting nipple change.

Many ordinary skin conditions can affect the nipple, and discharge has several possible causes. A clinician may examine the nipple, areola, breast, and armpit, then arrange imaging or a skin or tissue sample when the appearance persists or needs clarification.

Arrange a prompt appointment for discharge alongside a persistent nipple or areola change, especially when treatment for simple irritation has not helped. Note when the change began, whether it is one-sided, and whether fluid appears without touching the nipple. Avoid applying new creams immediately before examination unless prescribed.

Seek prompt medical review for bloody or clear discharge, a persistent crust or rash on one nipple, nipple inversion, a new lump, skin dimpling, or a change that is worsening. Urgent care is needed for rapidly increasing redness, warmth, swelling, severe pain, or fever.

Ask whether the nipple change needs imaging or a sample, what other causes are being considered, and how quickly results will be reviewed. Ask which changes should prompt an earlier appointment and how to protect irritated skin while waiting.
