How common is hot, red, swollen joint in pseudogout?

Pseudogout, also called calcium pyrophosphate crystal arthritis, occurs when crystals irritate a joint and cause an inflammatory flare. The joint may become warm, red, swollen, stiff and painful, with movement becoming uncomfortable. Knees and wrists are often involved, although other joints can flare. The symptoms can resemble gout or an infection inside the joint. A hot, swollen joint therefore needs a clinical assessment when the diagnosis is new or uncertain.

Rest the joint during the painful phase and avoid activities that load or repeatedly bend it. Support the limb in a comfortable position and use a cool pack wrapped in cloth for short periods if that feels soothing. Arrange medical advice about anti-inflammatory treatment; the safest option depends on kidney function, stomach health, other conditions and current medicines. Keep a record of when flares start and which joints are affected.

Seek urgent care if the hot, red, swollen joint comes with fever, chills, marked weakness, confusion, rapidly increasing swelling, or severe pain with gentle movement. Infection can produce a similar flare and should not be assumed to be pseudogout. Prompt help is also appropriate if you cannot use the limb or the symptoms are worsening quickly.

Ask how pseudogout will be distinguished from gout or infection, whether fluid from the joint should be tested, and which medicines fit your medical history. Ask what to do at the first sign of another flare.
