How common is fever in middle ear infection?

A middle ear infection often follows a cold or other upper respiratory infection. Pressure and inflammation behind the eardrum can produce earache, a blocked sensation, temporary muffled hearing, irritability in children, and sometimes fluid or discharge from the ear. Fever is one part of this pattern, not a way to diagnose the condition by itself.

Young children may pull at an ear, sleep poorly, cry more than usual, or eat less. These behaviours can have other explanations, so an ear examination is needed to confirm an infection.

Offer fluids and rest, and use suitable pain or fever relief only as directed on the product label or by a clinician. Arrange a medical review if ear pain is severe, symptoms are not improving, discharge appears, or you are worried about a child. Avoid putting objects or unadvised drops into the ear.

Seek urgent assessment for swelling, redness or tenderness behind the ear, severe headache, a stiff neck, confusion, balance problems, marked weakness, or a child who is difficult to wake. Prompt advice is also appropriate for fever in a very young infant or for someone with a weakened immune system.

Ask whether the eardrum shows signs of infection, how pain and fever should be managed, and what recovery changes are expected. If hearing seems reduced, ask when it should be checked again and what symptoms would need an earlier review.
