How common is fatigue in heart failure?

Fatigue is a common presentation of Heart Failure and may become more noticeable during ordinary physical activity. It can occur with breathlessness, swollen ankles, a cough or reduced exercise tolerance.

When the heart cannot pump as effectively, muscles may receive less oxygen-rich blood during exertion. Fluid build-up can also make breathing harder and disturb sleep, leaving a person tired during the day. Fatigue is not specific to heart failure: anaemia, rhythm problems, lung disease, infection and medicine effects can cause similar symptoms. A clinician considers the full pattern and checks for changes from your usual condition.

Take heart medicines exactly as prescribed and keep track of weight changes, swelling, breathlessness and activity tolerance if your care team recommends it. Balance gentle activity with rest, avoid sudden increases in exertion and follow advice about salt and fluids. Do not stop a medicine or change fluid intake without speaking with your heart-failure team.

Call emergency services for severe breathlessness at rest, blue or grey lips, chest pain, fainting, new confusion or coughing pink frothy sputum. Contact your care team promptly for rapidly worsening fatigue, new swelling, a sudden change in breathing or difficulty lying flat. Early review can identify fluid build-up or another problem.
