Fainting, seizures, and epilepsy

Epilepsy can cause episodes of impaired awareness or loss of consciousness. Some may look like fainting, while others involve stiffening, rhythmic jerking, unusual movements, tongue injury, loss of bladder control, or prolonged confusion. A typical faint may follow a warning such as warmth, nausea, sweating, or greying vision and recovery can be quicker, but appearances overlap.

The event should be described by someone who saw it if possible. Timing, posture, triggers, colour change, movements, injuries, breathing, and recovery are useful details. Clinicians may use the history, examination, heart tracing, and neurological assessment to decide what tests are appropriate.

What to do during an episode

Protect the person from nearby hazards, cushion the head, loosen tight clothing, and turn them onto their side after jerking stops. Do not restrain them, put anything in their mouth, or offer food and drink until they are fully alert. Note when the episode began and ended, then arrange medical review after a first blackout or any change from the usual pattern.

When urgent help is needed

Call emergency services if the episode lasts longer than expected, repeats without recovery, causes serious injury, happens in water, involves breathing difficulty, or is a first suspected seizure. Get urgent help for persistent confusion, new weakness, severe headache, pregnancy, or an episode that occurred during exertion.
