How common is facial pain and pressure in cluster headache?

Facial Pain and Pressure is a common presentation of Cluster Headache, often concentrated around one eye or one side of the face. Attacks can be intensely painful and may come with tearing, a blocked or runny nostril, eyelid swelling, or restlessness.

Cluster headache pain is usually strictly one-sided and centred around the eye, temple, cheek, or forehead. An attack may build quickly and follow a similar pattern each time. The affected eye may water or become red, while the nose on that side may feel blocked or run.

These features can overlap with sinus or other headache symptoms. The timing of attacks, their duration, associated eye and nose changes, and whether they recur in bouts help a clinician assess the cause. Facial pressure by itself does not establish a diagnosis.

Record when the pain starts, which side is affected, how long it lasts, and any eye or nose changes. Note possible triggers and medicines already taken. Sit somewhere safe and avoid driving while severe pain or visual symptoms are present.

Arrange a medical assessment if these attacks are new, recurring, or interfering with sleep and daily activities. Cluster headache often needs a clinician-directed treatment plan, so do not rely on repeated self-treatment without review.

Get emergency help for a sudden, maximal headache, new weakness or numbness, fainting, confusion, difficulty speaking, or a new loss of vision. Seek urgent assessment for severe eye pain with a red eye, fever and facial swelling, or a first severe one-sided headache. Prompt review is also appropriate when attacks are becoming more frequent or changing character.
