Why lactose can cause diarrhoea

Lactose is the natural sugar in milk. It normally gets broken down in the small intestine by an enzyme called lactase. When there is not enough lactase, undigested lactose reaches the colon, where bacteria break it down and produce gas. Water can also be drawn into the bowel, causing loose stools, cramps, bloating, and wind. Symptoms may depend on the amount consumed and the individual’s tolerance.

Diarrhoea after eating dairy does not prove lactose intolerance. Milk allergy, infection, coeliac disease, irritable bowel syndrome, and other causes can produce similar symptoms. A clinician can help distinguish these, especially if symptoms are new, severe, or associated with weight loss or blood in the stool.
