How common is coughing up phlegm in bronchiectasis?

In bronchiectasis, parts of the airways are widened and can have difficulty clearing mucus. This can lead to a regular productive cough and make chest infections more likely. The amount or thickness of phlegm may change during a flare-up. Fever, increased breathlessness, chest tightness, tiredness, or a noticeable change from your usual sputum pattern are details to report. A change in phlegm does not by itself identify the cause.

Follow the airway-clearance plan agreed with your respiratory clinician or physiotherapist. These techniques are intended to help move mucus from the lungs and work best when practised as taught. Stay hydrated unless you have been advised to limit fluids, and avoid smoke and vaping. Keep a note of your usual cough and sputum pattern so that new changes are easier to describe. Take prescribed inhalers or other medicines exactly as directed.

Contact your care team promptly if you have more phlegm than usual, a new colour or smell, fever, worsening breathlessness, or reduced ability to do usual activities. Get emergency help for significant coughing up blood, severe breathing difficulty, chest pain, fainting, blue or grey lips, or confusion. If you have a written action plan for flare-ups, use it and seek advice when symptoms do not respond as expected.

Ask what your normal sputum pattern is likely to be, which changes could indicate a flare-up, and when a sample should be tested. You can also ask for a review of airway-clearance technique and whether vaccinations or pulmonary rehabilitation are appropriate for you.
