How common is abdominal pain in ulcerative colitis?

Ulcerative colitis affects the lining of the large bowel. Inflammation can cause cramping, tenderness, urgency, diarrhoea, or pain before and during a bowel movement. Pain may settle when inflammation is controlled, but its pattern can vary between people. Abdominal pain has other possible causes, so the symptom alone cannot identify ulcerative colitis or show whether it is active.

Note where the pain is, when it begins, how long it lasts, and whether it occurs with diarrhoea, blood, urgency, fever, or reduced appetite. Arrange a clinical review if pain is new, persistent, or changing, especially if you already have ulcerative colitis. Keep taking prescribed treatment unless a clinician advises a change, and ask about a review if symptoms are breaking through.

Seek urgent medical help for severe or rapidly worsening pain, a swollen or hard abdomen, repeated vomiting, faintness, confusion, heavy rectal bleeding, or inability to keep fluids down. Contact your care team promptly for increasing bowel frequency, fever, or pain that is interfering with eating, drinking, sleep, or ordinary activity.
