Why it can differ with age

Asteatotic eczema, also called craquele eczema or winter itch, develops when the skin barrier loses too much moisture and protective oil. Age-related dryness can make the barrier more fragile, so bathing, dry indoor air, cool weather, friction and strong cleansers may trigger symptoms. The lower legs are common sites, although the arms and trunk can also be involved. Skin may look rough, flaky and fissured, with fine cracks that resemble a dried riverbed. Itching can disturb sleep, and scratching can create open areas that become irritated or infected.

Practical care

Use a gentle, fragrance-free cleanser only where needed and keep bathing brief with lukewarm water. Pat rather than rub, then apply a thick moisturiser while the skin is still slightly damp. Reapply after washing and whenever the skin feels tight. Soft clothing and protection from cold, dry air can reduce irritation. A clinician may recommend a medicated cream for inflamed areas and can review medicines or medical conditions that contribute to dryness. Avoid picking at scales or applying fragranced products to cracked skin.

When to seek care

Seek medical advice if cracks bleed repeatedly, pain increases, redness spreads, or the skin becomes warm, swollen, crusted or draining. Arrange a review when itching persists despite regular moisturising, the rash is widespread, or the appearance is not typical of dry eczema. Prompt assessment matters for older adults with diabetes, reduced circulation or a weakened immune system because broken skin may need closer care.
