Short answer

Horrible heartburn during pregnancy is often related to reflux and pregnancy-related changes, but persistent, severe or unusual chest discomfort should be assessed.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Typical sensation
Burning behind the breastbone, sometimes with a sour taste
Try first
Smaller meals and staying upright after eating
Arrange medical advice
Persistent symptoms, painful swallowing, repeated vomiting or unusual chest pain

What it may mean

Heartburn is a burning feeling behind the breastbone that can rise towards the throat, sometimes with a sour taste or food coming back up. Pregnancy hormones can relax the valve between the stomach and food pipe, while the growing uterus can increase pressure on the stomach. Acid Reflux is a related condition; Gastritis, Hiatus Hernia and Peptic Ulcer can also cause upper abdominal or chest discomfort, but symptoms alone cannot identify the cause. Obesity can increase pressure around the stomach for some people.

Notice whether symptoms follow large meals, spicy or fatty foods, lying down, bending or eating close to bedtime. Tell a clinician if you have pain when swallowing, food sticking, persistent vomiting or unexplained weight loss.

Ways to ease symptoms

Try smaller meals, eat slowly and remain upright after eating. Avoid lying down soon after food, and identify personal triggers rather than removing broad food groups without guidance. Raising the head end of the bed may help night symptoms; extra pillows alone can sometimes bend the body and worsen reflux. Wear comfortable clothing around the abdomen and keep fluids going in small, regular amounts.

Do not start antacids, acid-suppressing medicines or supplements without checking their suitability in pregnancy. A clinician or pharmacist can choose an option and explain how to take it. Acid Reflux Medication may be considered when lifestyle measures do not provide enough relief.

When to seek urgent care

Get urgent help for crushing or severe chest pain, breathlessness, fainting, sweating or pain spreading to the arm, jaw or back. Seek prompt medical advice for vomiting blood, black stools, severe abdominal pain, repeated vomiting, difficulty swallowing or dehydration. Pregnancy-related chest or upper abdominal pain should not be dismissed as heartburn when it is new, intense or accompanied by feeling very unwell.

Questions to take to an appointment

Could my symptoms be reflux or another stomach condition? Which foods, positions and medicines are suitable for me during pregnancy? What should I do if swallowing becomes painful, symptoms disturb sleep, or the burning does not improve?

Questions people ask

Why can pregnancy make heartburn worse?

Hormonal changes can relax the stomach valve, and pressure on the stomach can encourage its contents to move upwards, especially after meals or when lying down.

Can I take heartburn medicine while pregnant?

Some treatments may be suitable, but the right choice depends on your pregnancy, symptoms and other medicines. Check with a clinician or pharmacist before taking one.
