What changes after menopause

The ovaries produce much less oestrogen after menopause. This can alter the body’s temperature control, sleep, mood, vaginal tissues, bladder comfort and sexual response. Some people notice hot flushes or night sweats; others mainly experience vaginal dryness, lower sex drive or menopausal mood changes. Periods stopped is the expected milestone, but new bleeding after menopause needs assessment.

Lower oestrogen can also make vaginal and urinary tissues thinner and less well lubricated. This may cause discomfort with sex, burning or repeated urinary symptoms. Pelvic floor changes may contribute to pressure or a bulge, including pelvic organ prolapse. Symptoms vary widely and may change over time.

Ways to manage symptoms

Track symptoms, sleep, bleeding and possible triggers such as heat, alcohol, stress or spicy food. Light layers, a cool bedroom, regular movement and a consistent sleep routine may make daily symptoms easier to handle. Vaginal moisturisers used regularly and lubricants during sex can reduce friction; a clinician can discuss local or whole-body hormone treatment and non-hormonal options when appropriate.

Arrange routine care to review bone health, blood pressure, medicines and any ongoing vaginal or urinary symptoms. Do not start hormones or supplements without discussing personal risks, migraine history, clotting history and breast or womb conditions with a qualified clinician.

When to seek medical care

Seek prompt medical advice for any bleeding after menopause, especially bleeding that recurs, is heavy or follows sex. Arrange an assessment for a new pelvic bulge, persistent pelvic pain, unexplained weight change, pain with urination or vaginal symptoms that do not improve.

Get urgent help for sudden chest pain, severe breathlessness, one-sided weakness, fainting or a sudden severe headache. These symptoms need immediate evaluation rather than attribution to menopause.
