What rhabdomyolysis means
Rhabdomyolysis occurs when damaged muscle releases substances into the bloodstream. It can follow unusually intense exercise, crushing injury, heat exposure, prolonged immobility, seizures or certain medicines. Muscle soreness after exercise is common and does not by itself establish rhabdomyolysis, but severe muscle pain, weakness or swelling, especially with dark or reduced urine, is a warning pattern. Blood and urine tests help clinicians assess muscle injury and kidney function.
What to do now
Stop strenuous activity and seek urgent medical advice if rhabdomyolysis is possible. Do not try to exercise through marked pain or weakness. Do not force large amounts of fluid if you have heart or kidney problems or have been given fluid restrictions; a clinician should guide hydration. Take a list of medicines and supplements, and describe recent exercise, heat exposure, injury or illness. Avoid alcohol and further exertion until assessed.
When to seek care
Go to urgent care for dark tea-coloured urine, very little urine, worsening weakness, severe muscle swelling, confusion, fainting, chest symptoms or difficulty breathing. Seek prompt assessment after extreme exertion when pain is far greater than expected or does not improve with rest. Kidney complications may develop even when the skin looks normal, so waiting for symptoms to pass can be unsafe.
