When hives are not itchy
Hives are raised areas caused by temporary inflammation in the skin. Itch is common but not required: some people notice colour, swelling, warmth, or a shifting outline instead. Welts may appear after pressure, heat, cold, exercise, a medicine, food, or an infection, and a trigger may remain unknown. A hive usually changes or fades, whereas a mark that stays in exactly the same place, hurts, bruises, or scales may represent another rash.
What to observe and do
Take photographs in good light and record whether each mark moves, fades, or leaves a stain. Note recent medicines, foods, illness, new products, temperature changes, pressure, and exercise. Keep the skin cool and avoid rubbing. Do not take an antihistamine solely because a mark looks like a hive; ask a pharmacist or clinician whether treatment is needed and appropriate. Arrange a review if the pattern continues, returns often, or is difficult to distinguish from another rash.
When to get help
Seek emergency help immediately if the rash is accompanied by swelling inside the mouth or throat, trouble breathing or swallowing, wheezing, faintness, confusion, or sudden weakness. Contact a clinician promptly for fixed painful patches, blistering, bruising, fever, or significant swelling. Lack of itch does not remove the need to act on airway or whole-body symptoms.
