Why the ankles may be involved
Socks, footwear, elastic, sweat, heat, grass, insects or topical products can expose the ankle area to pressure or contact triggers. Hives can also occur as part of a reaction elsewhere in the body. They may itch, sting or feel warm, and separate welts can merge into larger raised areas. Hives tend to change shape and position rather than remain as one fixed patch.
A rash that is mainly painful, purple, scaly, blistered or associated with marked one-sided swelling is not typical of simple hives. Foot or ankle swelling can have causes that need an examination.
What you can do
Loosen or remove irritating socks and footwear, rinse away possible contact products, and keep the skin cool. Avoid scratching and take photographs before the appearance changes. Note whether the marks follow a shoe edge, pressure point or outdoor exposure. A pharmacist or clinician can help select an oral antihistamine treatment when suitable; do not share medicines or use someone else’s prescription.

Call emergency services for throat, tongue or lip swelling, trouble breathing, wheezing, faintness or rapidly spreading symptoms. Seek prompt medical advice for severe ankle pain, purple or bruised-looking spots, warmth with fever, sudden one-sided swelling, or difficulty walking. Arrange a routine review if the eruption repeatedly follows footwear or persists instead of shifting and fading.
