How heat-related hives behave
Heat-associated hives may appear as small itchy bumps or larger raised welts after warming up, exercising, bathing, or becoming stressed. They may improve as the body cools, but recurring episodes can fit a form of urticaria that needs assessment. Sweat, sun, tight clothing, and friction may each contribute. A persistent skin rash is not automatically hives. Swelling around the eyes, lips, or hands may be angioedema and can occur with or without welts.
Cooling the reaction
Stop strenuous activity, move to a cool place, loosen tight clothing, and use a cool damp cloth on affected areas. Sip fluids if you can do so comfortably, and avoid hot showers and vigorous rubbing. Record the activity, temperature, clothing, foods, medicines, and timing so a clinician can look for a repeatable pattern. An oral antihistamine treatment may be suitable for some people after pharmacist or medical advice. Do not self-start oral corticosteroid treatment.
When the situation is urgent
Call emergency services for breathing difficulty, confusion, faintness, throat tightness, or swelling of the tongue or mouth. Get prompt assessment if the reaction is widespread, returns often, or does not settle with cooling. Severe heat illness can also cause dizziness, weakness, or confusion and requires urgent care independently of the skin findings.
