What should you know about hip, knee and shin pain?

Short answer

Hip, knee and shin pain can arise from one overloaded movement chain or several problems, so the location, timing, and injury history guide assessment.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Reading the pattern

Pain that travels or appears in several regions may reflect a change in how you walk, run, stand, or load one leg. The hip can affect knee alignment, while foot and shin symptoms can make the hip work harder. Shin pain may involve muscles and the tissues around the lower leg, but a stress fracture is also a condition clinicians consider when pain is focused, worsening with impact, or present during ordinary walking. A symptom pattern alone cannot identify the cause.

Practical next steps

Pause the activity that provokes the pain and switch temporarily to comfortable, lower-impact movement if you can do so without limping. Note whether symptoms are on one side, whether they begin during activity or afterward, and whether there is swelling, tenderness, numbness, or altered strength. Supportive shoes and a gradual return can help when overload is involved. Do not repeatedly test a sharply tender shin with running or jumping. A clinician or physiotherapist can examine the hip, knee, ankle, and walking pattern together.

When to seek care

Seek prompt assessment for pinpoint shin tenderness, pain that is increasing despite rest, pain at night, or pain with ordinary weight-bearing. Urgent care is appropriate after a fall or twist when you cannot bear weight, a joint is visibly deformed, or swelling develops quickly. Emergency help is needed for a cold or pale foot, severe calf swelling with breathlessness, or new loss of sensation or strength.

Questions to ask

Ask which region appears to be driving the pattern, whether impact should be stopped, what examination or imaging is appropriate, and how to return to walking or sport without recreating the pain.

Questions people ask

Can hip pain cause shin pain?

Altered movement can transfer load along the leg, but separate knee or shin causes are also possible and need assessment.

Is shin pain always a stress fracture?

No. Several tissues can hurt in the shin; persistent or focused pain deserves clinical evaluation.
