Hip IT Band Syndrome: What to Know

Short answer
IT band syndrome is an overuse problem in which repeated movement irritates tissues along the outer thigh, often causing pain near the outer knee.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common trigger
- Repeated running, cycling, stairs or knee bending
- Rehab focus
- Hip strength, single-leg control and gradual loading
Where symptoms come from
The iliotibial band is a strong strip of connective tissue running from the outer hip toward the knee. Symptoms often appear during running, cycling, stairs or repeated knee bending, and may feel sharp or burning on the outside of the knee. Hip weakness, a sudden change in training, tiredness or altered running mechanics can increase irritation. Knee Pain is a common way this problem is noticed, even though the contributing movement may begin at the hip or pelvis.
Reducing irritation and rebuilding control
Temporarily reduce the activity that provokes symptoms, then return through shorter, easier sessions when walking and daily movement are comfortable. A physiotherapist can work on hip and trunk strength, single-leg control and running or cycling mechanics. Gait Retraining may help when foot placement or stride contributes. Gentle mobility work can be useful, but forcing a painful stretch across the outer knee is unlikely to solve the underlying load problem. Sports Massage may ease muscle tightness for some people, while exercise remains central to restoring tolerance.
When to seek care
Arrange an assessment if pain persists despite reducing the trigger, changes your walking, appears at rest or is accompanied by swelling, locking or giving way. Seek prompt help after a fall, sudden severe pain, fever, a hot swollen joint or new weakness. A clinician may consider other causes of outer-knee pain before discussing options such as a Cortisone Injection.
Questions for your doctor
Which activity is irritating the band? How should I adjust training? Which hip and trunk exercises fit my strength? Should my gait or footwear be assessed? What would indicate another knee condition?

Questions people ask
Is IT band syndrome only a knee problem?
Pain is often felt near the outer knee, but hip strength and movement control can influence how the band is loaded.
Should I stop all exercise?
Usually the provoking activity is reduced while comfortable fitness and guided strengthening continue, subject to assessment.
