Why both hips may hurt

The bursae over the outer hip can become irritated when they are repeatedly compressed or loaded. Symptoms on both sides may reflect shared factors such as sleeping position, prolonged standing, reduced hip strength, a change in activity, or walking mechanics that load each side. Pain may be aching or sharp with pressure and can spread a short distance down the outer thigh. The label is not confirmed by symptoms alone: tendon problems, arthritis, referred pain, or an injury can feel similar. A physical examination helps identify the sensitive structures and how the pelvis and legs move.

A balanced approach

Avoid lying directly on either painful side and consider a pillow between the knees. Break up long periods of standing or walking, but keep comfortable movement rather than complete rest. A physiotherapist can build a gradual programme for hip and trunk strength, review footwear and movement habits, and adjust exercises so neither side is overloaded.
