High thyroglobulin antibodies after thyroidectomy

Thyroglobulin is a protein made by thyroid cells. After thyroid removal, it may be measured during surveillance for thyroid cancer. Thyroglobulin antibodies can interfere with that measurement, making a reported thyroglobulin value less reliable. The antibody result is therefore considered alongside earlier results, neck examination, ultrasound, and any other tests your team recommends.

A single high result does not establish that cancer has returned. Changes over time may provide more useful information than one reading, but the meaning depends on your operation, any radioactive iodine treatment, the original tumour, and your wider follow-up findings.

Keep scheduled blood tests and imaging appointments, and bring previous laboratory reports if you change hospitals or clinicians. Ask whether the same laboratory method will be used, because comparing results is easier when testing is consistent. Take thyroid hormone exactly as prescribed unless your treating team changes the plan; the dose may be selected for replacement or for a particular level of thyroid-stimulating hormone.

Your clinician may repeat thyroglobulin antibodies, review the direction of the trend, examine your neck, or arrange ultrasound. Follow-up can also include discussion of thyroid cancer history and whether radioactive iodine therapy was used.

Contact your oncology or endocrine team promptly if you notice a new neck lump, increasing neck swelling, trouble swallowing, breathing difficulty, or a persistent change in your voice. Seek urgent help for sudden breathing difficulty or rapidly increasing swelling. These symptoms need assessment because they can have several causes, not because they prove cancer recurrence.

Ask what your antibody trend has been, how it affects confidence in the thyroglobulin result, which imaging is planned, and what thyroid hormone target is appropriate for you. You can also ask when the next blood test is due and which symptoms should prompt an earlier review.
