High fever but cold hands and feet

Hands and feet can feel cold when blood vessels narrow during chills, while the body keeps heat closer to its core. Fever and cold extremities can also occur when dehydration or a serious infection affects circulation. Pale, mottled, blue, or grey skin; unusual sleepiness; confusion; fast breathing; or a weak feeling are more concerning than cool fingers alone. Anaemia, iron deficiency anaemia, hypothyroidism, and peripheral arterial disease can cause cold hands or feet at other times, but they do not explain every episode of fever.

Compare both sides, notice colour and sensation, and check whether the person is alert and breathing comfortably. A thermometer reading should be interpreted alongside these signs.

Check the temperature and observe breathing, skin colour, alertness, and urine output. Offer suitable fluids if the person is awake and able to swallow. Use light layers and warm socks, but avoid hot water bottles or direct heat on numb skin. Do not massage a painful or unusually pale limb. If cold hands and feet are a recurring issue after the fever settles, arrange a routine assessment and mention fatigue, weight change, numbness, colour change, or walking pain.

Call emergency services for confusion, fainting, severe breathing difficulty, blue or grey skin, a cold clammy body, rapidly worsening weakness, or a person who is difficult to wake. Seek urgent assessment when one limb becomes suddenly cold, pale, painful, weak, or numb. Get same-day advice for persistent fever with poor drinking, very little urine, repeated vomiting, or a child who appears much sicker than the temperature alone suggests.
