High fever and red spots on the body

A fever can accompany viral illnesses such as chickenpox, COVID-19, or a common cold, although a red rash is not typical of every cold. Chickenpox often produces itchy spots that change into fluid-filled blisters. Skin that is hot, painful, swollen, or spreading may point to cellulitis. Exposure to animals or unpasteurised dairy products can be relevant when considering brucellosis. In children, cough, noisy breathing, or a barking cough may suggest croup or bronchiolitis.

The appearance matters: note whether spots blanch when pressed, feel raised, form blisters, or look like bruises. A rash that does not fade under pressure, especially with severe illness, requires urgent medical attention.

Measure the temperature with a reliable thermometer and record when the fever and spots began. Encourage frequent fluids and rest. Use loose clothing and avoid overheating. Do not scratch blisters, share towels, or give a child medicines that are unsuitable for their age. Take clear photographs of the rash as it changes, and mention recent travel, new medicines, insect bites, sick contacts, and vaccination history to a clinician.

Seek emergency help for trouble breathing, confusion, unusual sleepiness, a stiff neck, a seizure, blue or grey skin, severe weakness, or spots that do not fade when pressed. Get same-day medical advice for a rapidly spreading rash, severe pain, repeated vomiting, dehydration, or fever in a very young baby. Keep away from others if chickenpox or another contagious infection is possible until a clinician advises you.
