High CK levels and rhabdomyolysis: what to know

Creatine kinase is an enzyme found inside muscle cells. When muscle fibres are injured or break down, CK can enter the bloodstream. Rhabdomyolysis may follow unusually intense exercise, heat exposure, prolonged compression, seizures, trauma, or some medicines and medical conditions. Muscle pain, weakness, swelling, or unusually dark urine can occur, but symptoms vary. A CK result must be interpreted with symptoms, kidney tests, urine findings, and the pattern of change over time.

Stop strenuous exercise and contact a clinician promptly if rhabdomyolysis is suspected or a CK result is unexpectedly high. Do not try to manage a significant result with exercise or supplements. A medical team may review medicines and recent exertion, check kidney function and electrolytes, and advise fluids or observation based on the findings. Rest and ordinary hydration advice should follow the clinician’s plan, especially if nausea, vomiting, or kidney disease is present.

Seek urgent care for dark or reduced urine, severe muscle swelling or pain, marked weakness, confusion, fainting, vomiting, or an irregular heartbeat. Go immediately for breathing difficulty or chest symptoms. A person with symptoms after extreme exertion or heat exposure should not wait for symptoms to become severe.

Ask what may have raised CK, whether kidney function and electrolytes were checked, when repeat blood or urine testing is needed, and which medicines or activities to pause. Ask when gradual exercise can safely restart.
