What persistent hiccups can mean

A hiccup is an involuntary spasm of the diaphragm followed by rapid closure of the vocal cords. A brief episode can follow a large meal, fizzy drinks, alcohol, sudden temperature changes or swallowed air. When the reflex keeps recurring, irritation involving the stomach, food pipe, diaphragm or nearby nerves may be contributing. Acid Reflux can present with hiccups, particularly when there is also a sour taste, burning behind the breastbone or regurgitation. Gastritis can present with hiccups when stomach irritation occurs alongside upper abdominal discomfort, nausea or early fullness.

Persistent hiccups can also be linked with medication effects or another illness, so duration alone cannot identify the cause. A clinician may ask about recent procedures, medicines, alcohol, abdominal symptoms and neurological symptoms before deciding whether tests are needed.

Take small sips of still water, eat slowly and choose smaller meals until the episode settles. Avoid fizzy drinks, alcohol, very spicy food and rapid gulping if these trigger the spasms. Keep your upper body raised after eating and avoid lying down immediately. Do not try forceful breath-holding, swallowing objects or drinking unsafe mixtures. Record when the hiccups began, what you ate or drank, your medicines and whether sleep or meals are affected. Arrange a medical appointment because hiccups lasting days may need treatment aimed at the underlying cause; do not start acid-reducing medicine without checking that it fits your symptoms and other medicines.

When to seek care

Seek urgent medical help if hiccups occur with trouble breathing, chest pain, fainting, new confusion, weakness, severe headache, repeated vomiting or inability to keep fluids down. Prompt assessment is also sensible when the spasms prevent sleep, eating or drinking, or when they began after surgery or a new medicine. A clinician can distinguish a digestive trigger from a problem needing different care.
