Hemorrhagic cyst and ovarian cancer: what to know

Bleeding into an ovarian cyst can produce pain and a complex appearance on a scan. The appearance may change as blood is reabsorbed, so a clinician may recommend a repeat pelvic ultrasound or another assessment. Imaging helps describe the cyst’s size, location and internal features; it does not by itself provide every answer about cancer.

Ovarian cancer can present with symptoms such as persistent bloating, pelvic discomfort, early fullness or changes in bowel or bladder habits. Keep the imaging report and ask what follow-up interval is appropriate. Tell the care team whether pain began suddenly, whether you could be pregnant, and which medicines you take, especially medicines that affect bleeding. Depending on the appearance and symptoms, the plan may be observation, repeat imaging, blood tests or referral to a gynaecology team.

If cancer is suspected, further work-up may include surgery to remove an ovary or tumour profiling to guide treatment choices. Ask how fertility, hormonal function and future pregnancy plans affect the options. Genetic counselling and BRCA genetic testing may be discussed when family history or tumour features suggest inherited risk.

Seek urgent medical help for sudden severe pelvic pain, faintness, marked weakness, heavy vaginal bleeding, fever, repeated vomiting or a swollen, rigid abdomen. These symptoms can signal bleeding, ovarian twisting, infection or another urgent problem. Do not wait for routine follow-up if they appear.
