What care involves

Heat stroke can disturb temperature control, alertness, circulation and organ function. A nurse may assess airway, breathing, circulation, mental status, skin, temperature, urine output and the history of heat or exertion. The care team distinguishes heat stroke from heat exhaustion, infection, medication effects and other causes of altered behaviour. Treatment is adjusted to the person’s response rather than to skin temperature alone.

Immediate nursing priorities

Move the person to a cooler area, remove excess clothing and use wet cloths, fanning, cool water or another suitable cooling method. Keep the airway clear, position an unresponsive breathing person safely, and monitor breathing continuously. Do not give oral fluids to anyone with impaired alertness or unsafe swallowing. Clinicians may use IV Fluid Therapy and tests when indicated.

When escalation is needed

Emergency escalation is required for confusion, collapse, seizure, breathing difficulty, repeated vomiting, shock-like symptoms or falling responsiveness. Ongoing high temperature, worsening weakness or little urine also warrants immediate review. The nurse should report changes promptly and document cooling measures, observations, fluid intake and response so the medical team can act quickly.

Ask which observations are being followed, what findings would change treatment, whether blood or urine tests are planned, and how swallowing and fluids will be assessed. Before discharge, ask about rest, medication review, return to heat exposure and follow-up.
