Why hiccups may feel severe

A hiccup happens when the diaphragm contracts suddenly and the vocal cords close, creating the familiar sound. Large meals, fizzy drinks, alcohol, sudden temperature changes, laughing and swallowing air can set off a short episode. Forceful or continuing hiccups may irritate the diaphragm or the nerves that control it. Acid reflux and gastritis can be associated with repeated hiccups, especially when there is burning, nausea, belching or upper abdominal discomfort. Persistent hiccups can also disturb sleep, eating and speaking, so the effect on daily life matters even when the cause is not obvious.

Practical steps

Sit upright, take small sips of cool water and eat slowly once the episode settles. Avoid carbonated drinks, alcohol, very large meals and foods that clearly trigger reflux. Do not frighten yourself, hold your breath for a long time or swallow unsafe substances. Keep a brief record of when hiccups start, meals and drinks beforehand, medicines, and symptoms such as heartburn or vomiting. If episodes keep returning or prevent normal eating or sleep, arrange a medical review. Treatment depends on the cause; do not start reflux medication without appropriate advice.

When to seek care

Seek urgent help if hiccups occur with severe chest or abdominal pain, trouble breathing, repeated vomiting, confusion, fainting or new weakness. Arrange prompt assessment when they continue despite rest, make it difficult to drink, or cause dehydration or significant sleep loss.
