Hands, feet, and face swelling during pregnancy

Pregnancy changes fluid balance and can make feet, ankles, and fingers feel fuller, particularly later in the day or after standing. Warm weather and prolonged sitting may make this more noticeable. Facial swelling, swelling that develops quickly, or swelling that is much greater on one side should not be dismissed as routine. A maternity clinician may need to check blood pressure, urine, and other symptoms. Headache, vision changes, pain beneath the ribs, breathlessness, or feeling unusually unwell can add concern. Hand swelling itself does not diagnose a pregnancy complication, and an assessment is the safest way to interpret a new change.

Tell your midwife, obstetric team, or maternity unit when the swelling began and where it is occurring. Remove tight rings and shoes before swelling increases. Rest with your feet supported, change position regularly, and take gentle walks if your maternity team has said activity is suitable. Drink normally and avoid taking water tablets or other medicines unless prescribed. Note whether swelling improves overnight and whether one leg or arm is painful, red, or warmer than the other.

Contact your maternity team promptly for sudden swelling of the hands or face, a rapid change in swelling, or swelling with headache, flashing lights, blurred vision, upper abdominal pain, nausea, or feeling unwell. Seek emergency help for severe breathing difficulty, chest pain, fainting, or a painful swollen limb. Reduced fetal movement should be reported through your maternity service without waiting for the swelling to settle.
