How Hand, Foot and Mouth Disease Starts

The first phase can look like an ordinary childhood viral illness. A child may be tired, clingy, warm, off their food, or complain that the throat hurts. Mouth ulcers can then appear, followed by small red spots or blisters on the palms, fingers, soles, or other skin areas. The sequence is not identical for every child.

Because painful mouth lesions can be hidden at first, refusing favourite foods or drinks may be an early clue. A rash pattern and the child's overall condition help a clinician distinguish this illness from other causes.

Take the child's temperature if they feel hot, encourage regular sips, and offer soft foods that do not irritate the mouth. Keep the child home from close-contact activities while they feel unwell and follow local nursery or school advice. Wash hands carefully and clean frequently touched surfaces.

Do not scratch, squeeze, or peel blisters. Note when symptoms began, whether the child is drinking, and how often they urinate; these details are useful if an assessment becomes necessary.

Seek prompt advice if a young child will not drink, is urinating much less, has persistent vomiting, or becomes unusually sleepy. Emergency help is appropriate for breathing difficulty, a seizure, severe confusion, or a child who is difficult to wake. Arrange a consultation when the diagnosis is unclear or symptoms are getting worse rather than settling.
