Growth plate fracture of the arm

A growth plate is a softer area near the end of a growing bone where length develops. A fall, direct blow, or twisting force can injure it around the wrist, forearm, or upper arm. Pain, swelling, bruising, and difficulty moving the arm are common clues, although the injury may not be obvious from the outside. An examination and imaging help the clinician assess alignment and decide whether the growth plate or nearby bone is involved.

Keep the arm still in the position that feels most comfortable and support it with a sling or folded cloth if available. Use a wrapped cold pack for short periods to help swelling; do not place ice directly on skin. Remove rings or tight items from the injured hand before swelling increases. Do not straighten the arm, test its strength, or allow sports until assessed. Treatment may involve Fracture Casting when the bone is suitably aligned, while some displaced injuries need Fracture Fixation Surgery to restore and hold alignment.

Arrange prompt medical care for any suspected growth plate injury. Go to urgent care immediately if fingers are blue, pale, cold, numb, or difficult to move; pain is severe or rapidly worsening; swelling is tight; or the arm has an unusual angle or open wound. A child who cannot use the hand or arm after an injury should be assessed even if swelling looks modest. Follow-up matters because changes in movement, alignment, or growth may appear after the initial injury.

Ask which bone and growth plate are involved, whether alignment is stable, what immobilisation is needed, when follow-up imaging is planned, and when school, play, and sport can resume.
