Groin pain and diarrhoea in females: what to know

Diarrhoea can cause cramping, bowel urgency, and pain that is felt low in the abdomen or groin. A stomach infection, food-related illness, or another bowel problem may be involved. Pelvic causes, urinary conditions, and menstrual pain can overlap with bowel symptoms, so location alone is not enough to identify the source.

A separate hip or muscle problem is more plausible when pain is reproduced by walking, lifting the knee, or rotating the hip and does not track with bowel symptoms. Hip impingement, a hip labral tear, and hip osteoarthritis can all produce groin discomfort, but diarrhoea calls for attention to abdominal and pelvic symptoms too.

Replace fluids regularly and choose simple foods if tolerated. Note stool frequency and appearance, abdominal or groin location, fever, vomiting, menstrual timing, urinary symptoms, and possible pregnancy. Rest from strenuous exercise while unwell. Avoid anti-diarrhoeal medicine if there is blood in the stool or a high fever unless a clinician advises it.

Contact a clinician if symptoms persist, recur, or make it difficult to drink. A clinician may need to assess the abdomen, pelvis, urine, and hip depending on the pattern.

Seek urgent care for severe or worsening one-sided pain, a rigid or swollen abdomen, fainting, confusion, repeated vomiting, blood or black stool, marked dehydration, high fever, or possible pregnancy with pelvic pain or bleeding. Sudden severe pain with weakness or inability to stand also needs urgent review.

Ask whether the pattern points toward bowel, urinary, reproductive, or musculoskeletal causes; whether testing is needed; and which fluid, food, and activity choices are appropriate while symptoms settle.
