What the swelling means
Water on the knee is everyday language for extra fluid in or around the knee joint, also called an effusion. It may follow an injury such as an ACL Tear or Meniscus Tear, irritation from Knee Osteoarthritis or Knee Bursitis, or inflammation from Gout. A Baker's Cyst can create fullness behind the knee. The appearance of swelling alone cannot identify the cause.

A clinician may examine the joint and consider imaging or fluid testing. Joint Aspiration can remove fluid and sometimes helps identify infection, bleeding or crystals; it is performed by a trained professional. Further treatment may include rehabilitation, medicine or management of the underlying condition.
When to seek care
Seek urgent assessment for a swollen, hot or red knee with fever, chills or feeling unwell. Get prompt care after an injury if the knee changed shape, locked, gives way or cannot bear weight. Arrange a review for swelling that returns, steadily increases, limits bending or remains unexplained.
