Gestational Hypertension and Preeclampsia: Understanding Practice Guidance

Gestational hypertension is high blood pressure that begins during pregnancy without signs of organ injury, while preeclampsia includes high blood pressure with evidence that pregnancy is affecting organs or the placenta. Clinical guidance helps maternity teams decide which checks are needed and when closer observation, medicines, or delivery planning should be considered.

A blood pressure reading is only one part of the assessment. Your clinician may review urine for protein, blood tests for liver and kidney function, platelet levels, symptoms, and the baby's growth and wellbeing. Gestational hypertension can remain uncomplicated, but it can also change into preeclampsia, so follow-up matters even when you feel well. The condition is related to pregnancy and may improve after birth, although blood pressure can need monitoring during the postpartum period.

Keep every antenatal appointment and take prescribed medicine exactly as directed. Record home readings only if your maternity team has shown you how to use the monitor, including when the reading was taken and any symptoms present. Do not start, stop, or alter blood-pressure medicine, aspirin, or supplements without professional advice. Rest when advised, but do not use bed rest as a substitute for assessment. Your team may arrange repeat checks, urine testing, blood work, fetal monitoring, or scans.

Contact your maternity unit urgently for a severe or persistent headache, vision changes, pain beneath the ribs or in the upper abdomen, sudden swelling of the face or hands, shortness of breath, chest pain, confusion, heavy bleeding, or markedly reduced fetal movement. These symptoms need prompt assessment because preeclampsia can affect the brain, liver, kidneys, lungs, or placenta. If you feel acutely unwell, seek emergency care rather than waiting for a routine appointment.

Ask which blood-pressure readings should prompt a call, how often you need urine and blood tests, whether fetal growth needs extra monitoring, and which medicines are suitable in pregnancy. You can also ask what symptoms require hospital assessment, how birth timing will be decided, and how long blood pressure should be checked after delivery.
